Students
will have to purchase their own uniform for
their externships which cost
approximately $25.00 to 40.00.
We will
accept Private grants and individual scholarship
financing.
A devised
Scheduled Payment Plan consist of: Special
arrangements can be made for the student to make two
installment payments of $750.00 prior to
entering their externship (clinical rotation).
No grades
or credit will be given until all financial
obligations have been met.

Certification:
To receive a certificate in our
program, a student must complete the requirements
described in the catalog.
Exceptions to this policy necessitated by a
change in course will be handled in such manner as to
avoid penalizing the student.
Admission Requirements

An applicant must have a copy of their
High School Diploma or GED Equivalent.
·
Post – Secondary education beyond High
School (Optional).
·
A valid drivers license or other form of
I.D. ( Birth Certificate or Passport )
·
Copy of Social Security Card
·
Hepatitis Vaccination or signed Waiver
·
Measles, Mumps, Polio, and Rubella
documentation
·
Agree to complete CPR requirement or
provide medical waiver
·
TB skin test
·
All applicants for admission must be 18
years of age or older by the enrollment date
·
All applicants must interview with the
Director of Education prior to the scheduled class date.
HEALTH
TECH INSTITUTE advises all applicants that good physical
and mental health is necessary for successful completion
of the phlebotomy program. In addition, a good
educational background in English, science and math is
recommended.
